a) a copy of the medical form

b) a copy of the patient's NHS GP Surgery "Medical Summary" which can be requested by contacting the NHS GP Surgery.

Please ensure to email these to us or bring them to your appointment. This is so we can confirm the patient's medical history as required on the medical form.
